The candidates shall submit their application on the prescribed form available in the prospectus, so as to reach the Registrar, Sri Guru Ram Das University of Health Science, Mehta Road, PO Vallah, Sri Amritsar-143501 by Regd. AD post or by hand. The application should be complete in all aspects and should be attached with the following documents:

Demand Draft of INR 2000 in favor of ‘Registrar, Sri Guru Ram Das University of Health Science, Sri Amritsar’ payable at Amritsar.
Attested copy of matriculation examination certificate indicating DOB.
Attested copy of 10+2 or equivalent mark sheet.
Migration Certificate from last attended board.
Aadhaar Card photocopy.
Punjab Domicile Certificate.
Two self-addressed envelopes stamped worth INR 30 each.
